We are delighted to announce we are hosting a Congenital Heart Disease Transition Day for young people aged between 13 and 17.
The event is for young people aged 13 and 17 who are affected by congenital heart disease and known to our services here at Royal Brompton & Harefield hospitals, part of Guys & St Thomas’s NHS Trust. The event will be held at the Chelsea Football Club at Stamford Bridge, London. There will be a series of talks and workshops to help you better understand your transition to adult care; how lifestyle issues affect your heart condition, and where to access advice, support and know your options. The day will also be a great opportunity for you to meet other young people who live with the same condition or similar.
The information is delivered in a relaxed and enjoyable environment, with some group work and an opportunity to do a walk around the iconic Football Stadium! Lunch and refreshments will also be provided for you throughout the day.

Please note that as you are under the age of 18, you must complete a medical consent form before attending the event. You will be sent the online form to complete once you've signed up.
